This study investigates quality of life and nutritional contributing factors for quality of life in esophageal cancer patients who received surgery. This will enable to provide fundamental references for nurse to perform the more effective intervention in patient care.
The subjects of the study were 100 patients who diagnosed with esophageal cancer and received surgery at cancer center in Gyeonggi province were selected for this study and date was collected from the first of march 2012 to eight of may 2012. To measure the quality of life, EORTC QLQ-C30(European Organization for Research and Treat- ment of Cancer Quality of Life Questionnaire) and QLQ-OES18(Esophageal cancer module) tool was used, which are translated to Korean version. SPSS(Ver.18) program was used for data analysis. Descriptive statistics, t-test, ANOVA, multiple regression were used for this study.
The results of study are as follows.
1. The mean scores of global quality of life were 57.16±18.79. Emotional function and Cognitive function are found to be the highest levels in functional scores. In symptom scores, the largest number of sample had experienced fatigue followed by financial difficultis, dyspnea, diarrhea and insomnia.
2. The quality of life related to general factors and disease specific factors.
1) In general factors for quality of life, age, the main nursing provider and financial problem are shown to be statistically significant both in patient's function scores and symptom scores. Gender, religion, living situation, marital status and the level of education did not have statistically significance in quality of life.
2) The time period since operation and ECOG performance status are shown to be the disease specific factors that contributing to quality of life, the method of operation and pre and post operation care brought statistically significance both in functional scores and symptom scores. Patients experienced most symptoms until 3 months since the surgery and their physical function improved after 6 months since the surgery. Patients with high physical activity status had the better functions and the less symptoms. Stage of disease and post operative complications did not have statistically significance in quality of life,
3. When we looked at nutritional factors which can affect quality of life, the difference in the amount of food consumption before and after the surgery had statistically significance. A group of participants who had weight loss before the surgery did not have significant change in quality of life,, while the other group where decreased weight after the surgery showed statistically significance in quality of life, The closer to normal in BMI indicates the better in functional scores and all other eating habits apart from mealtime showed statistically significance which means when patients have regular meal and eat similar amount of food before and after the surgery, they have better functional scores and less symptom scores.
4. This study performed mutiple regression to find out factors contributing to quality of life, The main factor affecting quality of life were ECOG PS and the time period after surgery. They predicted 13.3% of quality of life. In addition, The functional scores and symptom scores of questionnaire items of the most significant factors are 'Pain' and 'Nausea·Vomiting'. They predicted 22.9% of quality of life.
5. There was more than 10 point difference in functional scores and minimum of 6 up to 28.8 point disparity in symptom scores between esophageal cancer patients and general papulations. We found the younger patients had more obvious impairs compared to the general papulations in the similar age group. In particular, the disparity of scores in 'Social Function' and 'Financial difficultis' was 44.5 points and 67.2 points respectively.
In conclusion, The time period since the surgery, ECOG PS and the amount of food consumption are the significant factors contributing to quality of life, in esophageal cancer patients. Nutritional status have a significant effect on both functional scores and symptom scores, thus active nursing intervention regarding nutrient must be offered to patients.
This study did not fully discover the relationship between various factors and quality of life, since little previous study exists, however it can provide the general understanding of esophageal cancer patients which helps nursing staff to perform a differentiated care service.
