The purpose of this study was to investigate the effects of the intermittent traction and manual traction on L4-5 disc protrusion and extrusion. The subjects were assigned randomly divided into the protrusion group(n=16) and extrusion group(n=16). The protrusion group was subdivided into the intermittent traction group(n=8) and the manual traction group(n=8). The extrusion group was subdivided into intermittent traction group(n=8) and manual traction group(n=8). The intermittent traction group had applied traction-treatment for 12 times in first two weeks, and for 6 times in next two weeks of frequency. The time applied was 30 minutes. The manual traction group had conducted long-lever(stretch) manipulation or reverse long-lever(stretch) manipulation twice a week for total of 8 times during 4 weeks doing And for the flexion-distraction therapy, the group has applied 12 times for the first two weeks, and 6 times for the next two weeks. The time applied was 3 to 6 minutes per treatment. The straight leg raising test and quantitative muscle test were measured in 1day/pre, 1day/post, 1week, 2weeks, 3weeks, 4weeks and 12weeks. The Oswestry disability index were measured in 1day/pre, 1week, 2weeks, 3weeks, 4weeks and 12weeks. The disc herniation index was measured in 1day/pre, 4weeks and 12weeks.
The results were as follows:
1. The effects of the IT and MT for SLR of patients with L4-5 disc protrusion.
1) It showed that both groups were significant for linear curve in tests of within-subjects contrasts(p < .05).
2) Tests of pairwise comparisons on IT group were a statistically significant difference between 1day/post and 1week, 1week-2weeks, 2weeks-3weeks and 3weeks-4weeks(p < .05) and MT group were a statistically significant difference between 1day/pre and 1day/post, 1day/post-1week, 1week-2weeks, 2weeks-3weeks(p < .05).
3) Tests of between-subjects showed that the MT group compared to the IT group were a statistically significant difference in 1 and 2 weeks(p < .05).
2. The effects of the IT and MT for QMT of patients with L4-5 disc protrusion.
1) It showed that both groups were significant for linear curve in tests of within-subjects contrasts(p < .05).
2) Tests of pairwise comparisons on IT group were a statistically significant difference between 1day/post and 1week, 1week-2weeks, 2weeks-3weeks and 3weeks-4weeks(p < .05) and MT group were a statistically significant difference between 1day/pre and 1day/post, 1day/post-1week, 1week-2weeks, 2weeks-3weeks(p < .05).
3) Tests of between-subjects showed that the MT group compared to the IT group were a statistically significant difference in 2 weeks(p < .05).
3. The effects of the IT and MT for ODI of patients with L4-5 disc protrusion.
1) It showed that both groups were most significant for linear curve in tests of within-subjects contrasts(p < .05).

5. The effects of the IT and MT for SLR of patients with L4-5 disc extrusion.
1) It showed that significant for quadratic curve in the IT group and linear curve MT group in tests of within-subjects contrasts(p < .05).
2) Tests of pairwise comparisons on each group were a statistically significant difference between 1day/post and 1week, 1week-2weeks, 2weeks-3weeks, 3weeks-4weeks, and 4weeks-12weeks(p < .05).
3) Tests of between-subjects showed that were no statistically significant differences according to the treatment period(p > .05).

8. The effects of the IT and MT for DHI of patients with L4-5 disc extrusion.
1) Tests of pairwise comparisons on each group were statistically significant differences between 4weeks and 12weeks, 1day/pre-12weeks(p < .05).
2) Tests of between-subjects showed that no statistically significant differences according to the treatment period(p > .05).
The results of this study revealed that effects of IT and MT in protrusion and extrusion group had a linear curve following treatment period. It revealed that the MT group had faster time than the IT group in an increase of SLR angle and QMT, had no difference in ODI and DHI. Therefore, it positively recommended MT to protrusion group.
It revealed that between the IT and MT group had no difference in SLR angle, QMT, ODI. but confirmed that regressed in DHI following treatment period. Therefore, it is considered that treatment in the course of time had efficacy in the extrusion group.
