Colorectal cancer ranks at the second in the incidence of all cancers except thyroid cancer and is rapidly increasing because the development of technology of early diagnosis and treatment has improved the survival rate of cancer. fatigue is the most common symptom of cancer patients that has an influence on further resilience and than eventually affects the quality of life. Therefore, I attempted the study of fatigue, resilience, and quality of life in patients with colorectal cancer.
The purpose of this descriptive research study was to provide basic data of nursing intervention to improve the quality of life of patients with colorectal cancer by identifying fatigue, resilience and quality of life of them and determining the factors affecting the quality of life.
The population is 102 patients being of active inpatient treatment and a follow-up who were more than 20 years old and diagnosed with colon cancer at the University Hospital in Seoul, from the January 21, 2013 to March 22, 2013 and the medical survey of general characteristics and disease-related characteristics, fatigue resilience, and quality of life of them were conducted with the approval of the Ethics Review Committee (IRB No. C2012236: 931).
As research tool, Korean version of the FACIT Fatigue Scale Version4 was used in fatigue, the 25-Item Resilience Scale in resilience, the Korean version of the FACT-C Version 4 with the approval of use in quality of life. the collected data was statistically analyzed using SPSS 20.0 and was evaluated using the frequency and percentage, mean and standard deviation, t-test, ANOVA, Cron- bach's α, Pearson correlation coefficients, Stepwise Multiple Regres- sion Analysis, and Scheffe test.
The results of this study are as follows.
The analyzed results of the degree of fatigue, resilience, and quality of life of the patients were that fatigue score ranged from 0 to 52 points and appeared on average 19.29 (SD 12.85) points, resilience scores which ranged from 25 to 175 points on average 117.70 (SD 21.96) points, quality of life scores which ranged from 0 to 136 points on average 86.44 (SD 20.35) points, respectively. itemized quality of life was that physical condition was on 19.53 (SD 6.82) points, social / family status on 16.19 (SD 4.87) points, emotional state on 17.01 (SD 4.53) points, feature state on 15.65 (SD 6.04) points, and the average specific area of colon cancer on ​​18.06 (SD 4.88) points, respectively.
When the fatigue, resilience and quality of life according to general characteristics and disease-related properties of the patient were analyzed, fatigue showed statistically significant differences with marital status, level of education which he or she had received, primary caregiver, changes in body weight during the past one month, hours of sleep a day, exercise time a day, ECOG PS, cancer stage, pain or not, and treatment progress up to date. On the results of analysis of the correlation between fatigue, resilience and quality of life, that between fatigue and resilience, fatigue and quality of life was negative but resilience and quality of life showed a significant positive correlation.
Factors affecting the quality of life were identified that were fatigue, resilience, sleep hours a day, and ECOG PS and these were found to explain the quality of life in patients with colorectal cancer at a rate of 71.6% (Adjusted R2).
The results of this study, fatigue, resilience, sleep hours a day, and ECOG PS were identified as the main factors that affected the quality of life of patients with colorectal cancer and according to each factor, significant characteristics of the patients were identi- fied. Therefore, this study is expected to present the basic data for development of nursing interventions to improve systemic and professional individual counseling, to relieve fatigue and enhance resilience and then improve the quality of life of patients with colorectal cancer.
On the basis of factors, ECOG PS of colorectal cancer patients and sleeping time a day, that affect the quality of life have been identified, follow-up studies which identify the quality of life of patients with colorectal cancer according to ECOG PS will be needed and to improve the quality of life by proper sleeping time, the development of an exercise program for cancer patients and verification its effectiveness be suggested.
